Residential Hospice (Hospice Palliative Care at HHHS)

 

Hospice Palliative care at HHHS is a partnership between Haliburton Highlands Health Services (HHHS) and SIRCH Community Services. The designated Hospice Palliative care bed at the Haliburton Hospital provides end-of-life care in a home like setting, providing privacy, dignity, comfort and support to the patient and family. And at the same time, medical and nursing care is on-site. The goal is to assess the patient and family’s needs and provide the support required to manage the emotional journey experienced at end of life.

This warm and inviting environment encourages families to make themselves feel at home, use the kitchen to make refreshments or sleep overnight near the patient.

A Team leader and a team of 4-5 volunteers make up the Hospice Care Team from SIRCH.  The Care Team is available for meaningful support, respite and comfort. Support is as minimal or as intense as the client and family wish or need. They will also help patients and families connect with other resources like spiritual care and bereavement support.
